Paper forms slow down field operations. Formict’s form builder lets you create digital forms that workers fill out on their mobile devices — with photos, signatures, GPS stamps, and conditional logic built in.

Form Builder Basics

Navigate to Forms → New Form to open the visual designer. Drag and drop field types to build your form:

  • Text — single line or multiline input
  • Number — numeric input with optional min/max validation
  • Select — dropdown with predefined options
  • Checkbox — boolean toggle or multi-select checklist
  • Photo — camera capture or gallery selection
  • Signature — touch-based signature pad
  • File Upload — documents, PDFs, images from device
  • GPS Location — auto-capture current coordinates

Conditional Logic

Fields can show or hide based on other field values. For example:

  • Show “Damage description” only when “Equipment condition” is set to “Damaged”
  • Show “Follow-up required” photo field only when “Issue found” checkbox is checked

This keeps forms concise — workers only see fields relevant to their situation.

Attaching Forms to Jobs

Once created, forms can be attached to jobs. Workers see assigned forms in their mobile job detail view. Submissions are stored with the job record and accessible from the web dashboard.

Offline Support

Forms work offline. If a worker is in a basement or rural area without signal, they can still fill out forms, take photos, and capture signatures. Data syncs automatically when connectivity returns.

Draft submissions are persisted to the device — if the app crashes or the worker switches tasks, their progress is preserved.

Compliance Use Cases

Common form templates in field service:

  • Safety checklists — pre-job safety inspections
  • Equipment inspections — condition assessment with photo evidence
  • Work orders — parts used, time spent, follow-up needed
  • Customer sign-off — signature confirming work completed
  • Incident reports — document issues with photos and GPS location

All submissions are timestamped and include the worker’s GPS coordinates, creating an audit trail for compliance requirements.
